The following are required at Registration Night.
____ PARENT AND PLAYER to sign and receive all necessary paperwork and player passes
____ 2012-2013 MEDICAL RELEASE FORM – completed, signed and NOTARIZED.
Form is available at www.fcrichmond.com, Forms (left side of home page)
*** Notaries MAY NOT be available onsite.
_____ College Brochure Information Sheet – U17 Magic, U18/19 Magic, U17 Mystx, U18/19 Mystx
_____ Scholarship Forms – form is updated on the website, to be used for families in need of financial aid help
_____ 1st Payment – for players that selected Mail-in-Check option for their first payment, these payments must be received by Registration Night…you may bring the payment to registration night.
_____ TWO (2) CURRENT PICTURES CUT TO SIZE - ONE INCH BY ONE INCH
Pictures must be in color, show full face and head. . Small or oversized pictures will not be accepted. Players must be dressed appropriately --no bathing suit pictures please!
_____ Cut picture to 1” x 1” size prior to registration night
_____ Write player’s name and team on back of photo
_____ CREDIT CARD INFORMATION FOR TOURNAMENT HOTELS
*** Parents will be asked if they need hotel accommodations, prior to using card information for tournament hotels.
_____ PROOF OF AGE / BIRTHDATE
Returning U11-U19 Players DO NOT need to provide Proof of Birth as current 2011-2012 Player passes issued by VYSA/US Youth Soccer will be used.
U10 Magic/Mystx players and, all players that DID NOT PLAY ON A FC RICHMOND U10-U18 Magic, Mystx, or Metro team in 2011-2012, must bring ONE OF THE FOLLOWING ITEMS THAT IS REQUIRED TO VERIFY PROOF OF AGE
Official/Original Stamped Birth Certificate (COPIES NOT ALLOWED)
Or
Passport (expired is acceptable, no copies allowed)
· Issued by the state/country in which the player was born.
· Photocopies of original birth certificates, baptismal and/or hospital birth certificates MAY NOT be used.
· All travel soccer clubs are required by US Youth Soccer to obtain an original birth certificate for verification by the league/state registrar. Please note that this is not an FC Richmond rule.
** Please note, we will not keep the Birth Certificate or Passport during the summer. Ryan Molloy, Director of Operations with the club, will verify the proof of birth at registration night.
